Those closest to Leanne Lapsin are in disbelief, after the mother-of-four's trip to the doctor turned to tragedy.
Ms Lapson lost consciousness while getting an injection at the Medisense Health Care Centre in Taree, NSW on July 31.
9NEWS understands authorities will investigate whether there was an air bubble in the syringe.

"It's not something I would wish on my worst enemy," friend Jane Howden told 9NEWS.
"One day we were fine, the next day our whole lives were turned upside down."
It's understood Ms Lapsin's death is being treated as a medical misconduct case.
Staff at the Medisense Health Centre have told 9NEWS the doctor, who's now under investigation, was an independent contractor and no longer works there.
It's not the first time a staff member from the private health clinic has been investigated. Ms Howden says her main focus now is to get justice for Leanne, and help look after her four children, the youngest of which is just two-years-old.
"They have to start again, you can't replace their mother."
The family friend is now fundraising to help support Ms Lapsin's family at this difficult time.
